Reset Password with a Reset Token

If you have not provided your email address during enrollment and have not verified your account, you must remember your valid username. To get a password reset token, contact your practice.

  1. On the logon page, click I have my password reset token.
  2. In the Forgot Password page, do the following:
    1. Type the token number provided by your practice.
    2. Type your username.
  3. Click Submit.

    Note: If you have not registered your email address, type your email address, and click Submit. If you do not wish to provide email address, select I do not wish to provide an email address.

  4. On the Reset Password page, do the following:
    1. Type your password.
    2. Re-type your password.
  5. Click Submit.

    The NextGen Enterprise Patient Portal home page opens.
